South Texas Surgical Hospital is a 33-bed facility with 5 operating rooms, 3 emergency department treatment rooms and the attached ambulatory surgical center (Outpatient Surgery Center) has 6 operating rooms.

South Texas Surgical Hospital is currently looking for a Registered Nurse Comprehensive Joint Replacement Patient Care Coordinator and Employee Health– FT

Responsibilities:

The primary responsibility of the Comprehensive Joint Replacement Patient Care Coordinator and Employee Health Nurse will be to guide and coordinate patients through the entire continuum of care. The Patient Care Coordinator will collaborate with the CJR team and post-acute providers to advocate for, supervise, and support patients through the entire episode of care to promote better outcomes and prevent unnecessary readmissions and complications during the 90 day recovery period. The Patient Care Coordinator will be responsible for collecting program outcome data and evaluating the effectiveness of the Total Joint Program. A strong focus will be placed on patient and community education/outreach, as well as hospitality and patient satisfaction.

  • All employee health related facility functions
  • Collaboration with the interdisciplinary team to ensure specific CJR outcomes are met
  • Develop CJR protocols, policies, and procedures with the collaboration of the interdisciplinary team using evidenced based guidelines to ensure targeted bench marks are met.
  • Monitor the CJR programs quality performance
  • Seek to obtain national quality awards recognitions. TJC joint certification, Health grades 5 star rating for Total Joint care.
  • Round on all total joint patients daily
  • Coordinate mandatory participation in pre-operative education and discharge class attendance for the total joint population.
  • Develop and maintain CJR patient data base.
  • Develop and maintain all MC 30 day readmission data base
  • Monitor overall patient, staff, and physician satisfaction.
  • Facilitate total joint pre-operative, inpatient, and outpatient care through ongoing clinical evaluation and management in collaboration with the entire patient care team.
  • Function in the capacity of the total joint Customer liaison.
  • Develop and maintain a system for the transmission of total joint replacement patient information to primary care providers including assistance with making post-operative appointments.
  • Assist with staff education and continuous performance improvement efforts relation to the total joint program.
  • Acts as a patient advocate and support to patients and families through the continuum of care.
  • Assist with budget planning.
  • Participate in marketing activities/outreach speaking engagements regarding the total joint program.

*Qualifications:

  • Demonstrates eligibility for employment in the U.S.
  • Graduate of approved school of Registered Nursing with current and valid licensure with Texas State Board of Nursing
  • Prefer a minimum Baccalaureate Degree in Nursing with a background in case management.
  • Minimum of two years clinical experience is required with an orthopedic background.
  • Knowledge of State, Federal and Accrediting Body regulations and standards related to healthcare.
  • CPR certification valid upon employment.
  • Demonstrated skills in verbal and written English communications for safe and effective patient care and to meet documentation standards. Spanish a plus.
  • Is aware of professional nursing standard and performs in accordance with them.
